NAMES

Names on will be ALL CAPS, but we understand so​me names need a lower case letter.  Please help us by typing your name in ALL CAPS unless you need a lower case letter such as McDONALD, DiNOLFO, LaSORTA.  Please do not type McDonald, Dinolfo, LaSorta, Smith, Johnson as we then need to guess what letters need to be upper vs lower case.

NUMBERS

Please do not add a leading zero to the number unless you want the leading zero, for example...  02 vs 2 , we will print the zero if you type it. 

If you have not been assigned a number by the organization, write 'NA' in the # section and we will confirm your # before placing the order.
